چکیده
Abstract Funding Acknowledgements Type of funding sources: None. Introduction Risk stratification in Brugada syndrome (BrS) is challenging. Cardiogenic syncope, family history sudden cardiac death and ECG markers are useful variables determining patient risk. Invasive risk sometimes needed those with an intermediate phenotype. Implantable loop recorders (ILR) may be considered patients no inducible ventricular arrythmias during programmed stimulation (PVS). Objective The aim this study was to retrospectively evaluate the outcome BrS referred for PVS ILR implantation. Methodology Demographic, clinical follow-up data seen clinic were analysed from hospital databases. A p vale <0.05 significant. Results 129 have been diagnosed since launching inherited arrhythmia service (53.5% males). 15 (high patients) had ICD (n=13, 10.1%) or PM (n=2, 1.6%) implanted because a arrest, arrhythmias, high grade AV Block cardiogenic syncope. 26 (20.2%) placed category, (65.4% Patients offered electrophysiology (EPS/PVS) depending on indication (Figure 1). Most male (65.4%), mean age 41.8±17.7 years at time stratification. 6 likely definite pathogenic mutation (n=5 19.2% SCN5A, n=1 3.8% MYBPC3). This group median Shanghai score 4.0±0.9. Gender (p=0.429), (p=0.246), genotype (p=0.273) shanghai (p=0.746) did not differ between strategies. (n=12, 9.3%) EPS/PVS offset. Ventricular arrhythmias induced 1 (8.3%), then implanted. rest day. EPS (n=14, 10.9%) initially insufficient grounds proceed EP study. (n=25, 19.4%) followed up 39 ± 21 months. monitoring identified actionable events 3 (12.0%) patients. One (4.0%) pacemaker sick sinus syndrome. Two (8.0%) recorded non-sustained arrhythmias. Both subjected EPS/PVS, one ICD. None who any significant follow up. Conclusion implantation has role stratifying do fulfil criteria referral. also keen invasive assessment EPS.

Syncope is a common, often perplexing clinical problem causing emotional stress to patients, their families, and often, the physicians caring for them. In a large, 26-year prospective study of men and women ages 30 to 62 years, 3% of men and 3.5% of women experienced at least one syncopal episode (1). That study excluded adolescent and elderly populations. Syncope has been reported in up to 47%...
متن کاملBrugada syndrome: Controversies in Risk stratification and Management
In the 18 years since the first description of Brugada Syndrome in a small series of cardiac arrest survivors it has become evident that there is a marked spectrum in phenotype and prognosis. An internal cardiac defibrillator (ICD) is the only established therapy but is associated with significant morbidity. A number of registries have published their data, but risk stratification, particularly...
متن کاملClinical diagnosis and risk stratification in patients with Brugada syndrome.
In patients with Brugada syndrome (BRS), especially those who are asymptomatic, preclinical diagnosis and risk stratification are vital to the prevention of the fatal ventricular arrhythmias. The initial optimism that a diagnosis of BRS could be made simply on the basis of distinct electrocardiographic (ECG) changes (see the following text) has been tempered, both by a high incidence of false-p...
